This Notice of Privacy Practices (“Notice”) describes how Mindfulous Integrative Health Clinic (“we,” “us,” or “our”) may use and disclose your Protected Health Information (PHI) as defined by the Health Insurance Portability and Accountability Act (HIPAA) and the California Confidentiality of Medical Information Act (CMIA). We primarily serve California residents ONLY in the US. International patients are welcome for consultations, but prescription services are currently limited to California residents.
*We are not able to see medicare/medicaid patients at this time.
We do not currently accept insurance. However, we can provide you with detailed invoices that you may submit to your insurance provider for potential reimbursement. We cannot guarantee that your insurance will cover any portion of the services.
Our membership includes:
All visits with the nurse health coach are reviewed by the medical doctor. These fees afford our patients more expanded access to our providers including unlimited messaging in between visits, access to same day care subject to availability, and prioritized processing of prescriptions, labs and imaging ordering, referrals, form completion, and other administrative tasks related to patient care.

We offer personalized, concierge-style, holistic care with improved access and more regular visits compared to traditional healthcare settings. Our integrative approach combines the best of conventional and complementary medicine to address your unique needs and goals, with a healthcare team helping you stay on track and adjust your goals over time.

You can easily cancel or reschedule appointments up to 24 hours before your visit through our patient portal. Cancellations or no-shows with less than 24 hours’ notice will incur a $150 fee.
Same-day visits are for established patients with urgent, non-emergency needs. Please note that these visits are subject to availability and are not for emergencies. In case of an emergency, always contact your nearest emergency department or call 911. You can request a same-day visit through the patient portal.
All our services are provided via telemedicine. Visits are conducted by video using a HIPAA-compliant electronic health record (CHARM Health) and video calling platform.
Our patient portal is a secure online platform where you can book appointments, message your providers, and access your medical records, including visit summaries, prescriptions, and other information.
By clicking on Book Visit and selecting the “follow-up” visit type for the provider you’d like to see. You can also schedule, reschedule and cancel visits through the patient portal.
