2. Complete your enrollment which will require paying applicable fees with a credit card.
3. Book your appointment and we will confirm it within 3 business days.

*Please note enrolling as a patient is a requirement for booking appointments. 

For Returning Patients:

2. Complete appointment request.
3. We will confirm your appointment within 3 business days.
